Changing Pathways: How to leverage evidence-based practice to improve treatment outcomes for OUD

Medications for OUD (MOUD) are the most effective treatment for OUD and yet there is still heavy reliance on traditional withdrawal management (aka “detox”) protocols in inpatient settings. Carelon’s Changing Pathways (CP) model focuses on incorporating MOUD induction into withdrawal management care with a warm transfer to community providers instead of relying on medical taper. The three essential components of the CP model are (1) MOUD education (2) MOUD inductions for individuals choosing this pathway; and (3) warm transfers to a community provider allowing these individuals to seamlessly continue their MOUD post-discharge. Peer specialists play an essential role in this model. CP has been associated with significant better outcomes than the traditional withdrawal management protocols including lower AMA rates, decreased readmission rates, better connection to care rates, and improved adherence to MOUD resulting on significant reduction in OD rates and decrease in service utilization post discharge. During this panel we will review the Changing Pathways model, steps for a successful implementation, and outcomes. Provider champions who have adopted this model will talk about their experience, lessons learned, and impact they have seen on treatment outcomes.
